CCM Health Welcomes Paula Tofte, Psychiatric Mental Health Nurse Practitioner

CCM Health is excited to welcome Paula Tofte, MSN, APRN, CNP, PMHNP-BC. Paula will join their Behavioral Health team and begin seeing patients in November 2026. She brings more than 15 years of experience in advanced psychiatric care, rural behavioral health and community mental health.  She is also board certified as a Psychiatric-Mental Health Nurse Practitioner through the American Nurses Credentialing Center.  

For Paula, a career in mental healthcare is more than treating symptoms. It’s about building relationships, understanding each patient’s needs and helping them achieve a better quality of life. She finds fulfillment in supporting patients through their mental health journeys and celebrating the progress they make along the way. 

Having lived in rural communities most of her life, Paula understands the importance of having access to mental health services.  “Being able to serve the community I’ve called home the past 30 years gives me the opportunity to provide much-needed mental health services to our rural area,” said Paula. 

Paula was drawn to CCM Health’s team-based approach to healthcare. She believes that working together to address both physical and mental health needs allows providers to better support patients and their overall well-being. 

Outside of work Paula enjoys spending time with her husband of 20 years, Brian, where they care for five horses, three dogs and two cats.  She enjoys horseback riding, camping and taking walks in the country.  Most of all she loves spending quality time with her 12 grandchildren, family and friends.
