At CCM Health, we’re always looking for ways to improve the care we provide and make your experience even better. That’s why we’re excited to announce a new partnership with Professional Research Consultants (PRC) as our patient experience survey vendor, starting July 1, 2025.
You might be wondering—what does that mean for you?
It means that after your visit to CCM Health, you may receive a short survey asking about your experience. These surveys can come by text message, email, or phone call. While the way you receive the survey might feel familiar, what’s new is how we’re using that feedback.

After a visit to CCM Health, you may receive a survey from PRC by:

The survey is completely confidential and voluntary—but every response helps. Even if you’ve shared feedback before, we encourage you to do so again. Each visit is different, and your input matters every time. If you are called for a patient feedback survey, the caller ID will be identified as “CCM Health/PRC.”
