CCM Health Receives Health Care Home Certification

The Minnesota Department of Health (MDH) has certified CCM Health as a Health Care Home, a program that strives to increase care coordination and collaboration between primary care doctors, clinicians, and nurses with resources to support whole person care and facilitate the broader goals of improving population health and health equity.

MDH’s Health Care Home program focuses on improving the quality, experience, and value of care that patients receive. CCM Health patients can work with one of its care coordinators who will focus on connecting health care patients and their families with wellness, prevention, self-management, and community resources.

Health Care Home also offers benefits in other ways. Minnesota’s Health Care Homes provided $1 Billion in savings to Medicaid and Medicare from 2010 to 2014 through connecting patients with care and services while also preventing gaps in patients care.

“Achieving this certification make us proud to know we are on the right track to provide patients with access to quality care and resources in the communities CCM Health serves,” Kas Ridl, CCM Health Clinic Nurse Manager, said. “Our Care coordinators are helping connect those points in medical services and the other services patients need to help them remain independent and healthy.”

“This accomplishment confirms CCM Health patients have more opportunities to receive team-based, coordinated, patient-centered care,” shared Barb Kleman, Clinic Director at CCM Health. “This is the kind of ‘total care experience’ patients are looking for and that can improve many areas of well-being in our patient’s lives.”

The Health Care Home certification means that CCM Health met all the rigorous requirements to provide high-quality patient and family care. It also means that the organization will continue its work to continue improving the quality and cost of health care it provides patients.
