CCM Health Earns Recognition from the American Diabetes Association
The American Diabetes Association® (ADA), the nation’s leading organization committed to fighting diabetes, has recognized CCM Health’s Diabetes Education department through its Education Recognition Program (ERP).
The ADA’s Education Recognition Program certificate, which is awarded for four years, confirms that CCM Health’s diabetes education services meet the National Standards for Diabetes Self-Management Education and Support. This evidence-based, outcome-driven service is designed to help people with diabetes gain the knowledge, skills, and confidence they need to manage their condition and improve their overall health.

Diabetes self-management education and support, often called DSMES, helps people with diabetes learn how to manage blood sugar, make healthy food choices, stay active, take medications correctly, reduce risks, and solve everyday challenges related to diabetes. These services also support patients as they work with their health care team to set goals and make informed decisions about their care.
According to the Centers for Disease Control and Prevention, an estimated 40.1 million people in the United States have diagnosed or undiagnosed diabetes, representing about 12 percent of the U.S. population. Diabetes can lead to serious health complications, including heart disease, stroke, kidney disease, vision loss, nerve damage, and amputation. The ADA’s Economic Costs of Diabetes in the U.S. in 2022 report estimated the total cost of diagnosed diabetes at $412.9 billion.
