What is Telehealth?

Telehealth is a virtual clinic visit you can have wherever you are and have an internet connection.  Telehealth visits are secure, interactive audio and video communications in real time with you and your provider.  Telehealth visits mimic a physical visit at the clinic with your health care team but in the comfort of your own home. 

How do I schedule a visit?

You can schedule a telehealth visit with a CCM Health provider by calling 320-269-8877.

Where do I go for my visit?

Anywhere you have an internet connection. You are able to do your telehealth visit in the comfort of your own home, anywhere with low background noise and anywhere you feel is private and secure.

What technology is needed for a telehealth visit?

Any smartphone, tablet or computer with speakers, microphone, and a web camera may be used. 

What can I expect during a telehealth visit?

MyChart Telehealth Visits
As of September 2020, patients will use MyChart to complete telehealth visits.
Patients who have scheduled a telehealth visit need to log-in to their MyChart account for his/her scheduled video visit. A message on the Appointment Details page will indicate that eCheck-in must be completed before joining the visit.
Please click the eCheck-in button and complete the eCheck-in steps that may include insurance, payment, and possible eConsent.

Bluestream Health Telehealth Visits
You will receive an email or link to access Bluestream Health. Please click on the link to start your telehealth visit. Your telehealth visit will be just like a typical office visit you would experience in the clinic. In most instances you will see a nurse and then your provider. You will communicate directly face to face over your device in real time.

How will I be billed for my telehealth visit?

Telehealth visits are billed like a normal office visit.

What types of visits can be seen through a telehealth visit?

CCM Health Telehealth provides care for children and adults for conditions including:

• Chronic condition appointments
• Behavioral Health

• Animal/insect bites
• Colds/flu symptoms including fever, cough, sore throat, and earache
• Cold sores
• Headaches
• Minor injuries
• Occupational Therapy
• Physical Therapy
• Rehab Services
• Skin conditions and rashes (eczema, poison ivy/oak, athlete’s foot, sunburn, etc.)
• Sinus infections
• Sore throat
• Speech Therapy
• Stomach conditions (Irritable Bowel Syndrome, nausea, vomiting, diarrhea, etc.)
• Pink eye
• UTI (Urinary Tract Infection)
